Looks reasonable. Should be OSD and GPL compliant.
Highlights in reverse order.
#5 is, as noted, a GPL quote.
#4 is essentially a no-op clarification. It doesn't prohibit any
behavior which would otherwise be allowed, and I don't see it
conflicting with the OSD or GPL.
#3 is similar to #4, and looks heavily BSD influenced.
