Choosing an open source license

This discussion prompted me to throw together my thoughts on how to
choose an open source license.  There is nothing new here.  This is
basically my text version of a license selection wizard (I would have
put it on different pages, with buttons and animated graphics, but,
frankly, I've never liked those things).
