I don't think this really belongs in the license; it belongs in your 
contributor guide (for the project) and your terms/NCO/CLA (for a code 
repository).  I don't think putting it in the license has any legal 
effect, since to be covered by the license the contributor has to agree 
that their contributions are under the license in the first place.

Agree with Carlo that you could use an attorney to help you finesse the 
patent grant.  What you're trying to do is actually much more difficult 
than it sounds, which is why there's not already a license like this.
