For Approval: The Azure License

Hi,

I'd like to submit the Azure License for review. I am the author of  
the license, and wanted you to review it and make sure it abides by  
open-source standards.

This license is being used by all my current and (mostly) future open- 
source projects, as well as any third-party that wishes to use it.

I come from the world of Mac and iPhone software development, and I  
release code to be used mainly in commercial, closed-source software,  
as is common in this field.

I have previously been using either the BSD or MIT license, sometimes  
not even bothering with a license at all. But they do not satisfy me  
fully:

I need a short human-readable license which would be similar to the  
MIT, with a major differences:

It would require attribution in an appropriate place to be read by  
interested users of any derivative work (typically a closed-source  
app), such as the documentation, credits document or about box. (For  
example, a “Contains code by Kenneth Ballenegger” or “Special thanks  
to Kenneth Ballenegger” in the about box is what I'm looking for.)

It also includes a no-endorsment clause similar to BSD's.
