Seems straightforwards; speaking as a developer, the requirements to 
comply with this license seem clear.  I'll let the attorneys say whether 
the patent clause works, legally.

I'm not entirely sure that there isn't already a license that covers 
your conditions, but if there is it's not as short as yours.
