"Must exist in the documentation" != "must remain in force on the source code."
A requirement to reprint the license is quite different from the license
itself, which always remains in force unless the copyright holder
revokes it.

The requirement about copyright notice refers to now-obsolete U.S.
copyright laws that insisted that each separate document have its own
copyright notice.
